Physician of the Year
CRITERIA FOR NOMINATION
This award is presented annually to a North Carolinian physician who has demonstrated a philosophical commitment to assisting home health and/or hospice patients who wish to remain at home.
Please be sure to address the qualities of the nominated individual that best meets the following criteria:
Describe role of this physician with your agency and how this physician actively supports your programs?
Willing to share knowledge (in-services)
Consultative services
Review Policies, Procedures, and Protocols
Describe how this physician serves as an advocate for in-home services in the community.
How has the physician served above and beyond call of duty or made a difference in the agency? Describe how well they collaborate with staff.

Sherry Leigh Thomas
Nurse of the Year
CRITERIA FOR NOMINATION
This award is designed to recognize and reward a registered nurse with at least 3 years experience in home care, home health or hospice who has dedicated their career and professional contributions to the field of home health or hospice care and has the heart of a teacher. This very special category seeks to highlight a commitment, passion and selflessness to delivering quality care, professionalism and patient outcomes.

Kathie C. Smith
Career Advancement Award
CRITERIA FOR NOMINATION:
The Kathie C. Smith Career Advancement Award will recognize an outstanding individual in our industry who has taken the initiative to advance their entry-level degree, certification or training by continuing their education for a higher degree or certification in order to better serve home care, home health and/or hospice patients in NC.
Examples of an ideal nominee is a Personal Care Aide becoming a Certified Nursing Assistant (CNA); a CNA becoming an RN; an RN earning their BSN.

Nurse Aide of the Year
CRITERIA FOR NOMINATION
This award is designed to recognize CNA’s and NA’s in the home and hospice care industry. The nominee must have a minimum of one (1) year experience working in home care and/or hospice.
They must demonstrate the following:

A commitment to the industry by serving on internal agency committees, AHHC committees or community involvement;

An interest in professional growth through continuing education;
-
Excellence and commitment to patient care; and
-
Dedication to raising the level of professionalism.

Family Caregiver of the Year
CRITERIA FOR NOMINATION
This award is presented to a non-paid friend or family caregiver who goes above and beyond the call of duty to promote patient/client well-being and enhanced quality of life including but not limited to factors such as comfort, safety and independence.

Volunteer of the Year
CRITERIA FOR NOMINATION
This award is designed to recognize Volunteers in Home Care, Hospice, or Home Health. The nominee must have a minimum of one (1) year of experience volunteering in hospice.
They must demonstrate the following:

Considerable commitment in their relationship to home care/hospice, including role and scope of volunteer activity.

Notable impact by strengthening the home care, hospice or palliative care program or the lives of patients and families served.

Serve as an inspiration to others.
