Teresa has provided educational programs on OASIS assessment, ICD-9 and ICD-10 coding, agency communications and processes, documentation, quality outcome improvement, and care transitions for regional and state conferences.  With 35+ years of experience as Clinical Services and QI/Education manager with home health agencies (independent, hospital-based, and county health dept.), a hospital patient educator and quality improvement facilitator, and a program manager with Primaris, the Missouri QIO, Teresa brings comprehensive and contemporary knowledge of home care and quality improvement methods.  Teresa joined Selman-Holman and Associates in 2011, and is certified in OASIS competency (COS-C), Homecare Coding Specialist-Diagnosis (HCS-D) and Homecare Coding Specialist – Hospice (HCS-H).

Christina Nuqui is a Social Worker, earning her MSW from Dominican University in 2005. As a social worker, her experience includes TBI Case Management, Foster Care Services, Elder Care and Long Term Planning, and more recently Home Health and Private Duty. Christina has over 10 years of experience managing health care professionals and patients. She began her career as a brain injury case manager, which stemmed her interest in home care. She has held roles as Brain Injury Program Director, Client Care Manager, Branch Manager, Private Duty Manager, and Home Health Administrator. She has been successful in the licensing and accreditation of Home Health and Private Duty agencies by focusing on creating and implementing policies and procedures to achieve outstanding levels of compliance. Her passion is creating and improving standards of home care for elderly consumers and their families.

Diane Poole is Fazzi’s Director of Strategic Initiatives and a member of our firm’s senior leadership team. She is a seasoned home care and hospice executive with expertise in accounting, finance, operations, software development, informatics and education. She has more than 20 years of experience in our industry having served at the senior leadership level at Baystate VNA & Hospice, Holyoke Visiting Nurse Association and at Suncoast Solutions. A dedicated advocate for our industry, she has served as Treasurer for the Hospice and Palliative Federation of Massachusetts and on committees for the Home Care Alliance of Massachusetts and the National Association for Home Care and Hospice Home Health Financial Managers Association. She is a frequent speaker at both national and state associations.

Tammy Ross is the Senior Vice President of Professional Services at Axxess, the fastest-growing home healthcare technology company. She oversees the professional services division at Axxess with a focus on practical operational solutions for the home health, home care, and hospice sectors of the post-acute care industry using technology. During her time at Axxess, Tammy has implemented strategic directives and developed operational materials to educate the industry on important new initiatives. Prior to joining Axxess in November 2018, Tammy served as an executive consultant at Fazzi and Associates, where she used her years of home healthcare leadership experience to assist clients with clinical and operational strategic planning and development. She also oversaw operations for a multi-state home health provider and was instrumental in creating disease management programs that lowered the Medicare spend rate and improved 5-star ratings. Tammy’s expertise helped open the first Medicare-certified home health and hospice agency in the U.S. territories of Guam and the Northern Mariana Islands. She also worked closely with Congress to expand the Women, Infant, and Children (WIC) food program to military families stationed overseas. Tammy has extensive knowledge of private duty agency operations through her time as an agency owner. She opened a school to train Certified Nursing Assistants (CNA) and used those graduates to deliver specialty disease management programs for her private duty patients. The agency’s dementia program was a highly desired by referral sources, caregivers, and providers. Tammy is the co-developer of Maintenance Therapy a Palliative Care Approach, which is a rehab program based on the premise of the Jimmo v. Sebelius settlement, specifying that patients do not need rehab potential to receive therapy services.

Richard Rutherford served as a senior contributor since 2008 in a variety of roles with North Carolina based software, billing and services vendor SembraCare, Inc. He has over 20 years of hands-on experience with the varied billing, regulatory and general business challenges of North Carolina Medicaid community care providers. Richard frequently interacts with State officials and contractors and serves as an active member of DMA/DHB working groups. He is and acknowledged expert in and frequent speaker on
Medicaid PCS services. He founded and is President of LTSS Consulting, Inc., which assists Medicaid providers and contractors in the Long Term Services and Supports Industry, including Home Care Agencies and Assisted Living facilities, especially with their transitions to Managed Care.

Dr. Schmerge is a doctorally prepared nurse who has spent over a decade working as a front-line provider in post-acute care. Dr. Schmerge has focused her recent work on strategies placing the patient at the center of care, driving clinical outcomes, and improving overall value. She develops teams around innovation and inclusion and gives front line employees opportunities to influence organizational strategy and improvement. Dr. Schmerge is the Vice President of Population Health and Care Transformation for Advanced Home Care. In addition, Michelle is a practicing adult nurse practitioner (NP) specializing in the care of an aging population. Dr. Schmerge holds a B.S. in mathematics and an M.S.N. from Vanderbilt University and a Doctor of Nursing Practice from Yale University. She regularly volunteers with the North Carolina Nurses Association and was named the Joanne S. Stevens Political/Legislative Nurse of the Year for 2016. Dr. Schmerge, her husband, and two children live in Denver, NC, where they enjoy sharing time on the lake with family and friends.

Nick Seabrook is a Managing Principal and Founding Partner of BlackTree Healthcare Consulting, where he develops and implements the company’s strategic goals and initiatives. A Villanova University alumnus, Mr. Seabrook’s personal and professional life have been defined by passion. Whether cheering on the Wildcats, volunteering for a cause with his family, or advocating for home care and hospice agencies alongside industry leaders in Washington, he is consummately involved with his community. Notably, he is a Member of NJ Homecare Association Billing Managers Group and an Advisory Board Member of HHFMA, where he also serves as Chair for Young Professionals Committee and Chair for Payment and Reimbursement Committee. He has presented at numerous state conferences and several national conferences including the National Association for Home Care & Hospice Conference and Expo, the National Leadership Conference hosted by ElevatingHOME, and the Private Duty National Conference and Expo hosted by DecisionHealth. Mr. Seabrook’s participation in these and many other industry forums and events over the past 18 years have garnered recognition as a leading authority on solutions to the latest operational, clinical, and regulatory challenges. In 2011, after more than a decade of applying a consultative approach to improving agency operations, he co-founded BlackTree Healthcare Consulting to assist agencies in adopting optimized revenue cycle and clinical practices. In the seven years since its founding, BlackTree has served over 350 clients, including some of the largest agencies in the country. The company was recently listed on Inc. 5000’s Fastest Growing Private Companies for a third consecutive year.

Rob is Director of Data Analytics at Simione Healthcare Consultants, where he focuses on building solutions to manage, analyze and operationalize data resources related to benchmarking, cost reporting, market intelligence and financial risk assessment. Prior to leading collaborative activities with a variety of information technology companies for Simione, he served more than 10 years in financial consulting, and in corporate and tax accounting. A CPA in the State of New York, Rob has been instrumental as an interim financial executive for home care and hospice agencies and health networks, and in developing the firm’s industry leading benchmarking tool, the Simione Financial Monitor. While working for Simione, Rob has developed a great understanding of data resources to support effective collaboration among information technology vendors and providers of home-based care. Through his extensive work as a financial consultant and interim chief financial officer, he has worked closely with a wide variety of organizations to develop financial, operational, marketing and IT benchmarks that are critical to both their daily business and long-term financial planning. Prior to his years with Simione, Rob worked for Price Waterhouse Coopers, where he received the Chairman’s Award for excellence in teamwork. There, he served as a senior tax associate, gaining experience with financial accounting and reporting standards for corporate and partnership taxes, auditing, equity compensation, and tax attribute allowances. He graduated from Villanova University with a B.S. in accounting.

Dr. Heather Smith is the Director of Clinical Partnerships and Integration for Advanced Home Care. From 2015-2017, Heather was privileged to lead Advanced Home Care during a three year journey in collboration with CMS, as one of eight home health agencies across the nation who elected to rapidly learn and resdesign home based care through the participation of the BPCI-Model 3 program. Today she has been able to integrate organizational learnings of bundle payments, and works closely with all business segments of Advanced Home Care, to identify strategic partnerships with providers across the continuum of care in order to meet the needs of both the current and future models of healthcare delivery and payment. Heather received her Bachelor of Science degree in Exercise and Sport Science from the University of North Carolina at Greensboro, a Master of Science degree in Physical Therapy from Winston-Salem State University and obtained her Doctorate of Physical Therapy from Shenandoah University. Over the past fourteen years, she has practiced as a physical therapist across all settings of care and served patients across the lifespan. Heather was the recipient of the 2014 VNAA Clinician of the Year award and 2019 AHHC of NC Leslie Crswell Leadership award. Heather is an active member of the American Pysical Therapy Association, including the home health and geriatrics sections. She resides in Colfax, NC with her husband and son, along with their one year old beagle, Blaze. She enjoys spending time outdoors, gardening, shopping and being with family and friends.

Karen is a leading national home care and hospice consultant with over 40 years of experience as both an occupational therapy provider, regulatory manager and a consultant. She routinely provides clinical and operations consulting services to home care and hospice agencies and helps providers establish and maintain quality management practices by providing assessment, training, and consulting. Karen is widely known for her industry expertise and is sought as a speaker at national, regional, and state industry events.

Ellie Ward, BSN, RN works with Vidant Health to promote best practices in end of life care and advance care planning across northeastern NC.  She is a graduate of NYU and worked as a nurse in the ICU and ED in urban settings in Los Angeles and New York City before moving to Dare County, NC in 1996.  Since moving to NC, Ellie has worked in home health and hospice as a direct care nurse and later as the Nursing Director, before transitioning to work with Vidant in 2015.  She enjoys working in rural communities across eastern NC, with providers and staff, as well as community members and community stakeholders- helping align care at the end of life, with what matters most to each person.
